Case Management
Brick Capital operates two main case management programs for those experiencing homelessness.
The Permanent Supportive Housing (PSH) program under HUD’s Continuum of Care (CoC) initiative is designed to provide long-term housing solutions for individuals and families experiencing chronic homelessness, particularly those with disabilities.

Stability
Housing + Services
PSH combines permanent housing assistance (like long-term leasing or rental subsidies) with supportive services such as case management, mental health care, substance use treatment, and life skills training. The goal is to help participants maintain housing stability and improve their overall well-being.
Criteria
Eligibility
To qualify, at least one member of the household must have a disability. Many PSH projects also prioritize individuals who are chronically homeless, meaning they’ve experienced long-term or repeated homelessness while struggling with a disabling condition.

Back@Home Program
Progressive Engagement for Housing Stability
The Back@Home program is under the North Carolina Department of Health and Human Services and is a collaborative rehousing initiative for those experiencing homelessness. To stabilize in housing, households must have a combination of adequate income, affordable housing, and appropriate support and services. Back@Home addresses these needs through a combination of financial assistance and housing stabilization services, enabling households to obtain and maintain housing.
Built on successful permanent housing models, including rapid rehousing and permanent supportive housing, the Back@Home initiative further innovates these models by shifting from a traditional approach of centering on funding requirements to centering on the household’s needs and experience. Typically, the delivery of assistance is designed around siloed funding sources with different eligibility requirements, target populations, and allowable uses of funds. This leads to a system that is difficult to navigate with gaps in service, barriers to resources, and duplication of effort.
Back@Home follows a Progressive Engagement strategy used in homeless assistance programs to provide the minimum necessary support to help households exit homelessness while keeping resources available for as many people as possible. The approach is flexible, client-centered, and cost-effective, ensuring that assistance is tailored to each household’s needs without providing more than is necessary at first.
